Foundations of Team-Based Primary Care

Primary care is facing a crisis.  Canada’s population is aging, and chronic health conditions are now the leading cause of disability and death. While there has been significant transformation in the past 20 years, more than 6.5 M Canadians are without a primary care provider.   Interprofessional primary care teams are seen as a core solution to provide Canadians with increased access and ensure comprehensive and coordinated care across the lifespan (Canadian College of Family Physicians). Occupational therapists, as members of these interprofessional primary care teams, are poised to play an important role in supporting primary care across the country. Course overview The course provides occupational therapists with foundational knowledge about primary care and the services they can provide in this setting.  The module introduces learners to primary care, including definitions, core elements, equity-oriented primary care, models to support collaborative interprofessional practice and case examples.  This module has been collaboratively developed with six professions and designed to provide a common understanding of primary care for all interprofessional primary care providers.

Foundations of Team-Based Primary Care
Begin self-paced component package.
Begin self-paced component package. "Successful Interprofessional Primary Health Care Teams (IHP) work in an integrated manner, complementing one another’s expertise and skills to achieve a shared vision and to ensure patient’s goals are met. In successful teams, each member acknowledges and respects the abilities of their colleagues and is willing to take on responsibilities to help or contribute to others’ roles. This requires that everyone understand the extent of their knowledge and areas of overlap with others’ disciplines, respect others’ perspectives and acknowledge they can learn from others’ disciplines and expertise" (Association of Family Health Teams of Ontario, 2023).
